We want to thank you for visiting the Coastal Dental Associates website! Our goal is for your first visit to be one of comfort and acknowledgement of your personal dental needs.  

During your initial consultation, our experienced dentists, and skilled team will discuss the details of your treatment plan and help you make the best decision for your desired outcome. We will take the required time to properly diagnose your immediate dental concerns, review your past medical and dental history, and create a treatment plan that provides for your optimal dental health. With your cooperation, we will complete a thorough comprehensive dental evaluation.

The foundation of a long term relationship is established at your first visit to our practice.

Patient Registration Forms

FIRST VISIT

Your initial appointment will consist of an examination and review of your treatment options. Usually, treatment will not be started the same day as your comprehensive evaluation.

Your dental cleaning visit will be appointed after your Comprehensive evaluation as the Dentist will need to determine your specific dental needs and we will schedule accordingly.  

Please assist us by providing the following information PRIOR to your initial visit:

  • Any x-rays taken by a previous dentist, if applicable.
  • A list of medications you are presently taking.
  • Dental Insurance information 
    • Please have your dental insurance information ready when calling the office to make your dental appointment. 

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

******Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-RAYS

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility. Our office does require all patients of record to have an FMX (full mouth series of x-rays) on file
